You must be working or volunteering in a licensed child care enter or be a registered family child care provider.

• Photo ID
• High school diploma / transcripts or GED
• Letter of employment or volunteering at a licensed child care center, stating position title, date of employment, number of hours worked each week (must be on school letterhead and signed by the administrator)
• Family Child Care certificate of registration for providers only
• First Aid and CPR Certificate
