Regardless of your vaccination status, have you been in close contact in the last 10 days with: (1) Anyone who is known to have laboratory-confirmed COVID-19 OR (2) Anyone who has any symptoms consistent with COVID-19?
Close contact is defined as being within 6 feet of an infected/symptomatic person for a cumulative total of 15 minutes or more over a 24-hour period starting from 48 hours before the illness onset.
